Notice for Dr. Castro’s Patients
DR. MANUEL CASTRO HAS JOINED KIM BARIATRIC INSTITUTE
*Please note: Kim Bariatric Institute does not accept Medicare/Medicaid. New and Current patients of Dr. Castro with this type of coverage will need to visit The American Society for Metabolic and Bariatric Surgery (ASMBS) website to search for a provider that accepts Medicare/Medicaid.
Current Patients who would like to transfer to Kim Bariatric Institute to see Dr. Castro can request their records by completing the Record Release Form and send it to the address below. The records must be received by Kim Bariatric Institute before the appointment can be made.
**Please be advised that there is a fee for your medical records. The fees are listed below.
Once Kim Bariatric Institute receives the records they will contact you to schedule an appointment. At that time they will request that you complete a Medical History Packet, pages 5 thru 8 and page 11 do not need to be completed.
Current Patients who would like their medical records or have them sent to another provider, may request their records by completing the Record Release Form and send it to the address below:

Patient records are in storage. Once we have access to the records, we will print, mail or fax them to the patient or the designated provider.
Please be advised that it may take up to fifteen (15) business days from the date of receipt.
**Charges for Medical Records:
The charges are as follows: Twenty-five dollars ($25) for the first twenty (20) pages and fifty (50) cents for every page thereafter, along with a reasonable fee for the actual costs of copying, faxing, mailing, shipping or delivery. Payment must be received prior to records being sent and should be made payable to New Beginnings Surgical Group.
